Scholarship Programs :: Wishes 4 Me Physical / Occupational Therapist Scholarship

The Physical / Occupational Therapist Scholarship is to help individuals majoring in Physical Therapy or an Occupational Therapist Program with college expenses.

You must meet the following qualifications to be eligible:

- Must have and maintain at least C Average or 2.0 GPA
- College Major must be Physical Therapy or Occupational Therapy and documented with the college.
- Must have volunteered to work with disable individuals for a total of 100 hours within the past 12 months of receiving the scholarship.
- Participate in at least 2 qualified fund raisers for Wishes 4 Me.
- Interview with one of the Officer’s of the Foundation.


The following expenses that are included in the scholarship program are:

- Tuition
- Meal Ticket
- Program Fees
- School Supplies
- Dorm Fees
- Tutoring Fees


The following documentation must accompany your application to be eligible:

- Most recent High School or College Transcript
- A copy of the college application form is to be mailed from the college.
- A volunteer log must be provided with signature of volunteer supervisor and contact information to support all volunteer hours. The log must contain dates and hours worked each day, organization volunteering with, volunteer supervisor, volunteer contact information and description of volunteer duties.Volunteer supervisor will be contacted for verification.
(If you are reapplying after being a recipient in the current year, the volunteer hours required is 60 hours within the past 12 months of receiving the scholarship).
- 1000 Word Essay and the title is to be "How Do I Plan to Make a Difference".
- Nomination Letter is required.

The number of scholarships and the amounts of the scholarships are determined twice a year.

Additional Requirements:
If selected to be a scholarship recipient, there will be additional information required to meet government and foundation guidelines.
